Diabetes Education - Your Journey to Wellness
Our clients are adults with Type 2 diabetes and adults at risk for developing Type 2 Diabetes (pre-diabetes)
The Diabetes program involves individual appointments with the dietitcian and nurse, group classes covering many diabetes related topics and community information sessions.

Physician Diabetes Referral
Your Journey to Wellness - Pre-diabetes
This three hour group class, facilitated by an RD and RN, provides adult clients identified with Pre-Diabetes with information needed to make lifestyle changes to delay or prevent Type 2 Diabetes from developing. Topics discussed are: what is diabetes, risks and complications, healthy eating, physical activity, monitoring of blood glucose and self management. This class is repeated monthly, usually on the first Wednesday of each month. Call to register.
Your Journey to Wellness – Diabetes
This program consists of 2 consecutive weekly sessions and a follow up session based on need. Facilitated by an RN and RD, it provides adult clients identified with Diabetes with information needed to make lifestyle changes to manage Type 2 Diabetes. Topics discussed are: what is diabetes, risks and complications, healthy eating, label reading, physical activity, monitoring of blood glucose, foot care, medications, sick day management, and self management. This class is repeated monthly, usually on the second and third Wednesday of each month. Call to register.
Individual Counseling
These sessions are designed specifically to meet a client’s personal needs. One on one sessions with either the nurse or dietitian are held as well as team sessions where both educators meet together with the client. The objective of this service is to address individual client needs, goal setting and self-management skills that cannot be addressed in a group session. The client is assisted in developing personalized action plans which are reviewed at each visit. The initial visit is two hours and follow-up visits are one hour with the Registered Nurse and Registered Dietitian. Call to register.
Living Well With Diabetes
This program was implemented by the Durham Region Diabetes Network and has been entrusted to the Diabetes Team for presentations in Oshawa. Participants with Pre-Diabetes or Type 2 Diabetes and their family member attend once a week for five weeks to obtain knowledge on how to live a healthy life with diabetes. Most sessions are held at a host off-site facility. Dates are set as opportunities for sessions are received.
Oshawa Diabetes Support Group
This popular program is open to adults with Pre-Diabetes or Diabetes and their family members. This group meets on the first Tuesday of each month at the Oshawa Loblaws Superstore on Gibb Street from 7:00 – 9:00 in the upstairs kitchen. Monthly meetings provide a peer-to-peer supportive network where participants share ideas and concerns in self-management of their diabetes. The monthly educational topics and guest speakers are based on the requests of the participants in the group. Pre-registration is not required.
